Floral Park Music & Dance Policy

PAYMENT OF FEES:
Tuition must be paid by automatic bank account debit or Post Dated Checks. Tuition will be debited from your account on the 1st day of each month from September-July. The month of September is non-refundable. Bank debit payments are not accepted for in-person payment on a monthly basis. All charges will appear on your bank statement as being from Floral Park Music & Dance Center. Incidental fees such as music books or dancewear must be paid in cash.

NSF PAYMENTS:
I hereby authorize the school to electronically debit my bank account for the amount of any NSF paper check or auto-debit transactions plus a $25 NSF fee.

WITHDRAWALS & REFUNDS:

There is a two-month minimum for all lessons. One-month written notice from the first of the month is required to discontinue any classes. Withdrawal must be done in person and will not be accepted over the phone. Withdrawal must be done at the school office and not with the teacher. Withdrawal must occur within the first 7 days of the month.

To withdraw from class a parent or adult student must:
1. Inform school administration in person and
2. Complete and sign a withdrawal form provided by the school office

All automatic bank debit charges will stop after the one-month notice period. The Floral Park Music & Dance Center reserves the right to terminate lessons to any student without notice. In such, a case a refund for unused lessons will be given. There are no refunds for a six week or "other" program(s).

EXTREME WEATHER:
In the event of inclement weather, such as heavy snow (8 inches plus) and the school system is closed, the studio will be closed. There will not be any make-ups. If you are unsure of closure, please call the school.

SUBSTITUTIONS:
The school reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to teach classes. If a teacher is ill and the school cannot arrange a substitute any missed classes will be made up.

DANCE DRESS CODE:
Required dancewear and dance shoes must be worn to all classes. For your convenience, you can purchase the necessary dancewear at the main desk. Failure to wear required dancewear to class could result in students being asked to sit out the class. Repeated failure to wear required dancewear could result in termination of lessons. For safety reasons dancers are not permitted to wear jewelry. Long hair must be tied back.

MISSED LESSONS & MAKE-UP LESSONS:
Should you have the need to cancel your lesson, please call 24 hours ahead of time. No make-ups are given without notice. It is the responsibility of the parent to call during studio hours to schedule a make-up lesson within 30 days of absence.* This is contingent upon availability on the schedules. No refunds are given for missed lessons. (school activities such as baseball, basketball or other sports games etc. do not qualify for make-up lessons.) There are no make-up lessons for dance lessons. **July is the only month where the make-ups must be made within the month of July only. Make-ups during the month of July are contingent upon the availability on the schedules. *Six week programs: make-ups are only provided within the six weeks.

FOOD & SNACKS:
Please enjoy your snacks and drinks outside the studio.

INJURIES:
Parents, legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on school property resulting from normal dance or music activity or any other activity conducted by the students before, during or after class time.

HOLIDAY CLOSINGS:
The studio will be closed on the following major holidays ****

Thanksgiving Day
Christmas Eve through New Year's day (Dec 24 – Jan 1)
Good Friday
Independence Day
**** Floating holiday(s) will be posted.
